At Willoughby Park Anglican Church, we seek to provide a safe, caring and loving environment for the children and young people in our care. We would like to know a little bit about you and your family so we can be aware of any issues that may arise and deal with them with care and sensitivity. Authorisations and Expectations

  • I give permission for my child to attend all scheduled Willoughby Park Anglican Church children's and youth activities, unless I advise otherwise. This includes usage of the church buildings, rectory and Senior Minister's property as well as Willoughby Park (across the road from the church).
  • I authorise the Willoughby Park Anglican Church children's and youth leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child. All attempts to contact the parent should be made beforehand. 
  • If transport is required during an event, I give permission for licensed (Green P Plates or higher) and insured drivers, appointed by the church leadership, to drive my child. All safe ministry considerations will be observed. 
  • I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in my child being sent home and being temporarily or permanently prohibited from attending activities.
  • I will provide the leaders with any information relevant to the wellbeing of my child prior to him or her attending an activity.
  • I confirm that the information given in this form is true and correct, and will advise of any changes to this information.
