Permission form: Scavenger Hunt 7/3/25 (Base Camp)

At Willoughby Park Anglican Church, we seek to provide a safe, caring and loving environment for the children and young people in our care. On Friday March 7th, 2025 from 6pm-9pm we are taking our youth group (Base Camp) on a scavenger hunt in a contained part of the city. The activity will involve meeting at church, getting the bus into the city, buying dinner, completing various challenges around the area of Circular Quay and the Botanic Gardens (in a smallg group accompanied by an adult leader), buying Gelato, and getting the bus back to church. If you have any questions about this Base Camp event, please speak to Mike Snowdon. Please complete the form below to give permission for your child to come along.

Authorisations and Expectations

  • I give permission for my child to attend the Base Camp event "Scavenger hunt" on March 7th, 6pm-9pm.
  • I authorise the Willoughby Park Anglican Church youth leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child. All attempts to contact the parent should be made beforehand. 
  • My child is aware of their physical ability. If not, I have given details above. 
  • I will provide my child with an Opal card for transport on bus and sufficient money for them to purchase their dinner and dessert or will provide them with dinner they can take themselves. 
  • I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in my child being sent home and being temporarily or permanently prohibited from attending activities.
  • I will provide the leaders with any information relevant to the wellbeing of my child prior to him or her attending an activity.
  • I confirm that the information given in this form is true and correct, and will advise of any changes to this information.
